Dr.
Mazer is not a provider with any
insurance carriers. He does not accept
Medicare or Medical.
If
you would like to submit to your insurance
please ask us for a form or use your monthly
billing statement.
Statements are generated the first week of every
month. They are due, in full, upon receipt.
The
fee schedule is as follows:

Telephone calls greater than 10 minutes in

hourly session fee.
