Statements are mailed out at the beginning of every
month, and are due in full, upon receipt.

Established patients
seen for medication management are required
to schedule (at a minimum) one 30 minute
session every 3-4 months.

Pharmacy and Insurance
Insurance plans which require
lengthy calls for pre-authorization or mail in
pharmacy benefits that require more than 10
minutes of my time will be billed proportional
to my hourly rate.
Telephone calls and Emails
Telephone calls and lengthy email communications
that require greater than 10 minutes in
length to respond to will be billed at a prorated amount of the
hourly session fee.
